Reade Allen Ballenger, age 74, of Evansville, IN, passed away at 4:07 p.m. on August 16, 2018, at home.

Reade was born April 8, 1944, in Liberty, MO, to the late Dr. M. C. and Marjorie I. (Hansen) Ballenger. He earned a Bachelor of Science Degree from Indiana State University in 1970, Masters of Health Administration Degree from Indiana University School of Medicine in 1975, and Doctorate Degree from Ross University School of Medicine, formerly University of Dominican in 1982. Reade was a member of the American Medical Association, American Academy of Neurology, Southern Medical Association, Georgia Medical Association, Laurens County Medical Association, Physicians Associates of Middle Georgia, Inc., and American Association of Electrodiagnostic Medicine Alliance Health Group, Inc.

Reade was on the Board for Eligible Internal Medicine in 1986 when his residency was completed and for Eligible Neurology – Examination in 1998.

Reade held licensure in Georgia, Tennessee, South Carolina, and Virginia.

July 1990 – Present: Reade had a Private Solo Practice in Neurology in Dublin, GA, where he cared for patients, both adults and children, with neurological problems. He started and supervised a neurodiagnostic laboratory at Fairview Park Hospital where he had privileges. He also did limited general neurology due to shortage of primary care physicians in the area, initially – interest included EMG and EEG.

July 1987 – July 1990: Reade served as the Chief Resident in Neurology for the Department of Neurology at the University of Tennessee College of Medicine. His responsibilities included patient management in conjunction with attending and consulting physicians. Rotations in specialty services in pediatric neurology, neuropathology, EEG, and EMG. Throughout the program, he had patient clinic in which patients were provided continued care by the same physician. Reade continued to do emergency room work during residency.

July 1986 – July 1987: Reade was a Practicing Physician for emergency room, locum tenens, primary care physicians’ offices, and clinics in Georgia and South Carolina.

July 1982 – July 1986: Reade was a Resident for the Department of Internal Medicine for Quillen – Dishner College of Medicine in Tennessee. His responsibilities included primary care of patient diagnosis and treatment in conjunction with senior resident as well as attending and consulting physicians for twenty-four months. He participated in rotations in specialty services of Pulmonary Medicine, Hematology, Oncology, and Critical Care. As a Senior Resident, supervision and education of first-year residents and medical students was given at three area community hospitals and a Veterans Administration Medical Center. Elective rotation included Emergency Medicine, Laboratory Medicine, Radiology, Neurology, Infectious Disease, Gerontology, Dermatology, and Outpatient Management and Care. Throughout the program, he had an outpatient clinic where patients were provided continued care by the same physician.

April 1979 – July 1982: Reade attended Medical School at University of Dominica in the Commonwealth of Dominica.

August 1976 – April 1979: Reade was the Assistant Administrator for the South Bend Osteopathic Hospital in South Bend, IN. His responsibilities included the supervision of many ancillary services and special attention was given to the development and implementation of workable reorganizations in purchasing, personnel, respiratory therapy, medical records, and pharmacy. A major concern of this position was directed towards a multimillion dollar expansion and renovation of the facility. These responsibilities included preparation of applications for Certificate of Need, preparation for accreditation and Medicare, Medicaid, and state licensure surveys, and implementation of Delegated PSRO status.

January 1976 – August 1976: Reade was the Hospital Sanitation Consultant for the Indiana State Board of Health in Indianapolis, IN. His responsibilities included inspection of sanitary features in hospitals and submit surveys to determine compliance with applicable standards for hospital licensure as well as Medicare and Medicaid certification.

January 1975 – January 1976: Reade was the Administrative Assistant of Environmental Services for Pulaski Memorial Hospital in Winamac, IN. His responsibilities included supervision of dietary, housekeeping, laundry, maintenance, construction, payroll, and personnel. He achieved compliance with state and accreditation standards previously sighted as deficiencies in the Dietary Department. Reade supervised in-house construction of a new x-ray room, remodeling a structure into a physicians’ clinic and remodeling to accommodate an ambulance service. He also served as Acting Administrator during Administrator’s extended illness.

August 1970 – January 1975: Reade was Registered Environmental Health Specialist for the Indiana State Board of Health in Indianapolis, IN. He worked as a Sanitarian in the Division of Sanitary Engineering where emphasis was directed to drinking water quality and proper disposal of sewage and solid waste through education, assisting local personnel, and inspections.

Reade served as Clinical Assistant Instructor for a teaching appointment for the Department of Internal Medicine at Quillen – Dishner College of Medicine in Tennessee.

Reade’s hospital staff privileges include the following facilities: Fairview Park Hospital in Dublin, GA from 1990-1998, HCA Regional Hospital of Jackson in Jackson, TN from 1988-1990, City of Milan Hospital in Milan, TN from 1988-1990, Tift General Hospital in Tifton, GA in 1987 , McDuffie County Hospital in Thomson, GA from 1986-1987, Emanuel County Hospital in Swainsboro, GA from 1986-1987, Dwight D. Eisenhower Army Medical Center in Fort Gordon, GA from 1986-1987, Edgefield County Hospital in Edgefield, SC from 1986-1987, and Allendale County Hospital in Fairfax, SC from 1986-1987.

He is survived by his loving wife of 4 years, Nancy Jo (Boles) Montenegro; step-son, Sergio Montenegro II of Evansville, IN; sister, Barbara Smith of Louisville, KY; brother, Milton “Mark” Ballenger of Eagle, CO; and nieces and nephews.

He is preceded in death his parents, Dr. M. C. and Marjorie I. Ballenger and sister, Anne Marcia Buchanan.

No Funeral Services and Burial will be Private.
