David William Johnson, M.D., age 80, of Evansville, IN passed away unexpectedly on Wednesday, March 19, 2025, at St. Vincent Hospital – Evansville.

Dr. Johnson was born November 23, 1944, in Indianapolis, IN to the late Lowell “Bud” Johnson and Theodora “Theo” (Pfeiffer) Johnson. He was a member of Washington Avenue Church of Christ. David and Phyllis’ love story began 39 years ago after they met while she was working at Deaconess Hospital. They enjoyed working together and traveling whenever they could, both in the United States and in Europe.

Dr. Johnson loved learning and was quite proud of his educational accomplishments. In 1968, he earned a Bachelor’s Degree in Education from Indiana University. In 1970, he earned three degrees: a Master’s Degree in Education from Indiana University, a Bachelor’s Degree in Human Biology and a Bachelor’s Degree in Anatomy, both from National College.

He taught for a few years before continuing his education by studying Chiropractic Science at National College. In 1973, he earned his Doctor of Chiropractic Degree and worked as a Chiropractor in Indianapolis for several years.

His love for learning led him to study medicine, and in 1984, he earned his Doctor of Medicine Degree (M.D.) from the American University of the Caribbean. He spent his Residency in Physical Medicine and Rehabilitation at the Medical College of Virginia from 1984 – 1987.

Dr. Johnson served as Medical Director for Rehabilitation Services at Deaconess Hospital in Evansville, IN for 2 years; MDA Clinic Director at Deaconess Hospital in Evansville, IN for 1 year; Vice President of Southern Indiana Chapter of MDA, was the MDA Chairman of Medical Advisory Committee, and was the MDA Clinic Director at the Tri-State Regional Rehabilitation Hospital in Evansville, IN for 6 years; and has successfully run his current Private Practice in Pain Medicine since 1989.

Dr. Johnson’s Hospital Services over the years include: Deaconess Hospital Committees, Physical Medicine and Rehabilitation, Medical Education, Medical Records, and Medical Research. His past and current Memberships include: American Academy of Physical Medicine and Rehabilitation, American Medical Association, Indiana State Medical Association, Vanderburgh County Medical Association, American Association of Electrodiagnostic Medicine, American College of Occupational and Environmental Medicine, American Academy of Pain Management, American Board of Independent Medical Examiners, and American Society of Addiction Medicine. For the entire duration of his practice, he had Staff Privileges at both Deaconess Hospitals and St. Mary’s Hospital, now known as St. Vincent Hospital.

Dr. Johnson was licensed to practice medicine in Indiana, Kentucky, Virginia, and Michigan. He was Board Certified in Physical Medicine and Rehabilitation (Pain Management), Addiction Medicine, and Independent Medical Examiner. His four publications include: Alternative Medicine March 2005, page 69 “Indiana Doctor,” Web Search Indiana Doctor “Fibromyalgia,” Orthopedic, February 1994, page 108 & 109, and Erythema Occipitalis Traumaticum and Stork Bite.

Dr. Johnson loved his work, his patients, and his office staff. He faithfully worked until his untimely passing. He was loved and will be missed by all who knew him.

Dr. Johnson is survived by his loving wife, Phyllis Stilwell Johnson and their dog, Roxie; daughters, Natalie Holland (Chris) of Sellersburg, IN and Jackie Adams (Jerad) of Avon, IN; stepson, Matthew Allen Snyder of Newburgh, IN; and grandchildren, Eric Holland, Kinsey Holland, Morgan Adams, Brady Adams, Riley Adams, Matthew Scott Snyder, Jessica Snyder, and Christopher Snyder.

Dr. Johnson is preceded in death by his parents, Bud and Theo Johnson; and only brother, Thomas Johnson.

Funeral Services will be held at 4:30 p.m. CST, Saturday, March 22, 2025, at Washington Avenue Church of Christ, 4001 Washington Avenue, Evansville, IN with David Rogers and Alan Bush officiating. Burial will be Private in Oak Hill Cemetery.

Friends may visit from 1:30 p.m. until service time CST, Saturday, March 22, 2025, at Washington Avenue Church of Christ.
